Self Care Assessment
Self care is the practice of looking after your own health and mental wellbeing. Often times, these become regular practice and part of your routine.
This assessment is a starting point to evaluate different areas in your life and the attention you place on them. There are no right or wrong answers. This serves as a way to help you explore areas you may strengthen or maintain.
Label each with one of the below:
1: I’m happy with where I’m at with this activity
2: There is room for improvement
3: I would like to do this more frequently
Physical
____ I maintain a balanced diet (I eat things I enjoy while also incorporating the nutrients my body needs)
____I have a healthy relationship with food (don’t over or under eat)
____ I get my body moving regularly
____ I take care of personal hygiene
____ I get enough sleep
____ I attend medical care appointments (when sick or preventative: physicals, teeth cleanings etc.)
____ I don’t abuse alcohol or drugs or use it regularly to feel better.
Emotional
____ I talk kindly to myself
____ I talk about my problems
____ I allow myself to feel and reflect on my feelings
____ I allow myself to relax
____ I’m able to ask for help if I need it
Boundaries
____ I maintain a schedule that is manageable
____ I have a healthy relationship with time spent on social media (doesn’t cut into time needs in other important areas: relationships, work, school, spiritual)
____ I can say no to others
____ I communicate personal wants and needs
____ I don’t compromise my values for others
____I set limits with friends and family, work or school as needed
____ I respect the rights and values of others
Social
____ I have people I can talk to
____ I take time off
____ I participate in hobbies
____ I spend time with my romantic partner
____ I make time for friends
____I feel connected to a community
Spiritual
____ I have things in my life that give me/my life meaning
____ I am open to new experiences
____ I nurture my spirit with nature, music, or art (or anything that sounds appealing)
____ I look at what’s good about myself and others
____ I make time for self-betterment
